Thanks for the education OCMike. I think the point is though - shouldn't it be the Palliative Care Counselor helping Grandma and possibly her children in making the decision rather than a panel ?

As I understand it, it's a panel of Palliative Care case managers who review a patient's record to make a recommendation to the Palliative Care specialist, who advises the patient. You might think this is just adding a layer of bureaucracy, but the positive is that you get a standardized level of care (fewer physician errors) and as a result, a net cost savings. And remember, these are advisory boards. The final determination will still be made by the specialist and patient (or family).

By the way, "case management" has been a standard cost-saving measure in the industry for many years. The only difference is that in the past it's generally been done by one person...usually an RN that specializes in a particular field (but works for the HMO).

Yeah, it's a very new field of medicine, so I can understand a bit of the uneasiness with the topic. But I was really disappointed in the talking heads that ran with the "death panels" farce. There were a lot of legit angles to attack the non-reform bill and they go into spooky fairy tale mode? Sad. And I think the ridiculousness of that attack helped it gain traction.

Rog, healthcare has and will always be rationed. As one example, if two patients need a critical transplant and there's one OR available because the rest are in use, someone's going to be shit out of luck.

Want another? Need an MRI on a knee? Not if the HMO is overbudget in radiology for the fiscal year. They'll deny your referral, "lose" your paperwork and dodge you for a few weeks before finally referring you to six weeks of PT. By the time you're done with that, they'll be in the black again and you can have your MRI with contrast for all they care.

In the world of socialized medicine, if you want a certain medical procedure and the government healthcare system does not permit it, you don't get it (unless you can find another country that has a private health insurance system and doctors willing to perform the service for cash).
