I think pain management requires trust, and healing the whole person, not specific symptoms. I think pain management abuse occurs when the 90 other issues in a person's life are not addressed at the same time.

To name a made-up example: an under-employed 50 year old man who intentionally impregnates his 17 year old second cousin, while sleeping with other relatives and non-relatives, has more issues than the work-related back pain he's being seen for. Especially when he doesn't keep his lies straight to any of them. The back pain is real. So is his abuse of the painkillers he's given. How does one manage the healing of the whole person?

Another example: 30 year old military wife wants for nothing except stability, a spouse who isn't away for months at a time, and someone else to look after the children she loves but resents.
Is it boredom or guilt? Who knows. She's an addict, and so is the military wife who hooked her up on the side as a form of shared female bonding. How does one manage the healing of the whole person?
