Of course, it's a bit less risky when pilots make at least a minimal effort to familiarize themselves with the location of the fuel switch.

Fuel transfer valve failure after "hold short" engine run-up procedure. Common civilian check lists have you switch tanks before taking the active runway for takeoff. Passing a few hundred feet, the engine starves for fuel and no engine power. Of course, no idea what really happened, but that is possible.

Actually Indy has had at least three aircraft accidents, two of them involving dynamic rollover in a B206L4 and a B407 when landing in a field. He has either very bad spatial and somographic responses to visual input or he just doesn't understand the concept of basic aerodynamics where helicopters are concerned. As a pilot he sucks.

I've flown in and out of Santa Monica a few times from San Diego Montgomery and just looked at the satellite map to refresh my memory. Then I saw the video of the plane with sound before it ditched. I heard sound from the spectator but no sound from the aircraft while it was rapidly descending. He was around 1000' AGL at most, and wanted to do a brave maneuver to go back to RWY 3 (the 180 of RWY 21). He would have crashed hard/stalled trying to do so if he turned back.
The best move was to set it down on the golf course and stall with the tail down to hit the ground 1st. To his credit, the trees are very tall and to hit the fairway and walk away... is okay with that old relic.
My guess is still that is was fuel starvation since I heard no engine noise. If it was an engine failure, then you might hear mechanical failure. I wasn't there, so no idea if bacon fat clogged the fuel lines.

I don't know squat about fixed wings, but I'd think that with a long par 5 hole and the gear down, he might have been able to just land it power-off at minimal airspeed? Unless the turf was wet and the gear would sink in? These are things I don't have to concern myself with as I'd just do a zero-run on autorotation and touch it down like it was on the showroom floor. But that's just me. Then again, I almost gave my C152 instructor heart failure when on my first flight with him I tried to hover the plane on the numbers. Negative habit transfer and all that. :doh:
